Why is successful execution so difficult? How does one execute effectively? Most organizations fail to execute as effectively as they could. In hospitals, over 200,000 people annually die needlessly because of the failure to execute proven safety procedures. In business, more than 2/3 of all major initiatives (acquisitions, mergers, reorganizations, new product launches, etc.) fail to achieve their stated objectives. Globally, McKinsey found that only 7 out of over 1,000 large multinational firms financially out-performed their peers for any 5 consecutive years in the past decade. Jim Collins found that even among companies with "good" performance, less than 1% were capable of producing "great" performance.
